New Patient Registration

Please ask at reception if you would like to check whether you live within the catchment area. In deciding whether or not to accept a patient, the practice will not discriminate on grounds of disability or medical condition, age, appearance, race, gender, social class, religion or sexual orientation. If an application is refused the applicant will be given the reason for refusal in writing.

How to Register

You can either register online using the form below or come into the practice.

To help aid us in the registration process when attending we ask that you bring with you:

One photographic ID (passport, driving licence etc.) and one proof of residency, this can be any of the following: bank statement, utility bill (gas, electricity or water) council tax bill or land line telephone bills (we do not accept mobile phone bills). These must be dated within the last three months. These documents are required for each adult registering.

We as that you bring identification papers in the form of a passport/EU Card or Home Office Papers if you have not previously registered with a GP in the UK as this will help us register you for the first time.

Bringing proof of address and ID with you to register will help us match up your medical records and will make for a smoother registration process.

You will also need to complete either:

Online Medical Questionnaire For New Patients. This form is submitted electronically to the practice. We will then contact you to book a health check appointment. If you need an appointment to see a doctor please contact reception and an appointment will be booked for you. When you come for your first appointment we ask you bring in your proof of ID and address and hand to reception.

Or

A New Patient Health Check Form for 0-5 year olds. This form cannot be submitted electronically on-line. Please download the form and complete for all under 5’s and remember to bring your child’s red book and full details of immunisations when attending

You should also bring with you, a medical card (if you have one) if not please download form GMS1 form which should be completed and brought with you when you attend the Practice or you can send it via email to wfccg.leytonhealthcarepatients@nhs.net. If you have had a previous GP please provide their name and address and your NHS number.

Please complete all forms as thoroughly as possible as it saves time when attending the Practice.  An appointment will be given for you to see our Healthcare Assistant or Practice Nurse for a new patient health check

Please note: we are open till late Monday – Friday until 20:00 for patients that cannot attend during the day to register. Late appointments are also available for new patient health check appointments.

Please note that in some circumstances other documents may be required. Further information can be provided by our reception staff.
