If you’ve been off sick for more than 7 days

To request a sick note:

For all medical certificate or sick note requests please ensure that you have "self-certified" for ONE WEEK before asking for a extension. Please ensure you state that you have self certified for one week when you submit your request. Please submit your sick note request on the date it is required as sick notes will be given from the date of submission.

Medical certificates/sick notes will be sent out via email please make sure you include your email address in the eConsult request. 

If you’ve been off sick for 7 days or less

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.

Sick Note Policy

Your first week of sick leave is covered by the employers under the SSP regulations. There after the following applies for sick notes:

Condition Length of sick note

Cancer and other significant diagnosis such as SLE and MS

2 Months

Post-operative recovery

1 month, follow by 2 weeks at a time.

Max 6-8 weeks

Mental-ill Health

To be assessed by MHP.

Sick notes can be provided initially for 2 weeks then extend every 2 weeks

MSK Acute

2-4 weeks

MSK chronic

1-2 weeks at above
