The Practice is keen to develop the role and involvement of its Patient Participation Group.

Patient Reference Group - Have Your Say!

The Leyton Healthcare Patient Reference Group (PRG) would like to hear from you. The PRG is a virtual group set up with the aim of helping the surgery to improve services and patient experience by completing online surveys anonymously. The completed survey helps us to decide which areas of the surgery we should focus on improving.

Responses of the survey will be analysed and an action plan to address any issues raised will be published on the surgery website and will also be displayed in the surgery waiting area.

If you would like to join our virtual group to give and receive feedback about your views and experiences, please contact a member of the Practice Management team on 0208 430 8282 to express your interest.

We will add your details to a contact list and get in touch with you to gather your feedback; this is done mostly by e-mail.

If you are happy for us to contact you occasionally by email please click the link to open the sign-up form and complete all the fields.

If you prefer, you can download the sign up form as a pdf document, print it out, complete it and return it to the practice.

We will be in touch shortly after we receive your form. Please note that no medical information or questions will be responded to.

Many thanks for your assistance.
